• At-home meeting with the caregiver(s) and the person living with dementia to support a comprehensive, person-centered “All About Me” approach to care.
• Cognitive testing/reading assessment- reports provided
• 2 hours

Option #2- At-home Dementia friendly Design- $300

• Success strategies based on individual needs
• Create a supportive environment that provides the person living with the dementia the opportunity to remain as autonomous as possible.
• Dementia-friendly “room to room” design solutions
• 3 hours

Option #3- Family Education Sessions- $300

• In-person or virtual options available
• A guided presentation will be offered to help families better understand dementia and supportive care approaches.
• Families will take part in the education opportunity with the option to provide examples and challenges they face.
• 3 hours

Option #4- The Transition Package- $500

• Application assistance
• Retirement/Long Term Care side by side touring
• 2-week transition support into Care Setting
